  • No Cost Transformation Assistance from Strategic Interests
  • Enhanced Reimbursement Opportunities:
    • Medicaid PCMH Incentive: $6.00 per member per month
    • Medicaid FFS per Visit Add Ons: $29 professional claims, $25.25 Institutional Claims for PCMH 2014 Level 3 or PCMH 2017
