Large Regional Health Systems
NYS DOH Opioid Program – $300,000 awarded
SI helped negotiate an opportunity with the Bureau of Narcotics Enforcement, helped write the grant, manage the project, and contributed to the solution development successfully creating the first EHR integration in New York State with the state’s PDMP database.
Transformation Projects SHCFT – $85,400,000 awarded
SI helped write and manage the Statewide Health Care Facility Transformation grant applications for a series of grants to transform the health system at 6 different sites
Community Diabetes Collaborative $13,000,000 awarded
SI provided advisory, strategic and tactical expertise to convene a multi-stakeholder initiative including payers and state and county health departments to form a Quality Improvement Initiative for Diabetes Care.
Integrated Community Health Provider
Supportive Housing Initiative – $524,000 awarded
SI helped research and write ESSHI grant for multi-unit, mixed-income supportive housing for people living with HIV/AIDS and others at risk.
Academic Medical Center
Clinical Cardiovascular Research – $4,000,000 pending
SI provided advisory, strategic and tactical application review for a multi-year American Heart Association Strategically Focused Research Network grant to fund 3 to 5 years of research on understanding, prevention, diagnosis and treatment in a key area of focus. The project lead relied on SI to review and strengthen their highly clinical application documents totaling over 80+ pages.
Health Information Exchange
SI wrote the grant that formed a public Health Information Exchange and then helped further the their capabilities with the Health Education Assistance Loan 3 program that allowed the additional ability to do image exchange.
DSRIP Collaborative
Medicaid Redesign: SI has been instrumental in helping two of New York State’s Performing Provider Systems prioritize, allocate and manage funds for IT enablement, telehealth and transformation.
