New York eHealth Collaborative and Strategic Interests LLC are collaborating to sponsor an evening of food and drinks on April 9^th, from 5p to 7p, at Templeton Landing’s Skyline Room, to introduce the 2017 New York State Patient Centered Medical Home (“NYS-PCMH”) Model and what this program means for primary care practices in New York State. Our event will also feature a discussion of the value of health information exchange to primary care practitioners to given by western New York’s regional health information organization, HEALTHeLINK, and an introduction to population health initiatives in the area by the Western New York Population Health Collaborative. We welcome all primary care providers and practice managers to attend and learn about these new opportunities.
