New York State has aligned with NCQA for the NYS 2017 PCMH program. All New York based practices that previously achieved PCMH recognition (2011-2014) will now be required to achieve additional requirements by the next renewal period. Grant funded support services are available from Strategic Interests to assist practices with NYS PCMH 2017 renewal or new recognition.
